Re-Entry Services
Re-entry services is an approach for successful integration for our residents.We work with Delaware Department of Correction, Probation and Parole, the Department of Labor, Department of Health and Social Services, the Department of Education,
Sojourners’ Place, and the Re-entry participant.
The Re-entry Team at Sojourners’ Place works directly with each ex-offender to coordinate the transition plan of each person to ensure the appropriate services and supports are available and accessed post release. Transition plans are monitored and progress reports are maintained for each participant to monitor progress made and to identify any barriers, gaps, and resources.
The Re-entry Team at Sojourners’ Place serves as a liaison between interagency collaboration and service coordination. This coordination has been the key to successful reintegration of each participant.
The major component of Case Management at Sojourners’ Place includes assessment and support in the following areas;
housing, education, employment and training, substance abuse education, and mental health and wellness.
