Case-Managed Services
Acceptance into the Sojourners' Place residential program is a 4 step, client driven process. Prospective residents must call (302) 764-4713 to set up an appointment to be interviewed by one of our Case Managers.
Prospects should have photo ID and a Social Security card if possible. Additionally, each prospective resident should be prepared to discuss their current living situation, addiction history as well as other factors which may have contributed to their homelessness.
If our Case Manager determines that a prospect is a viable candidate for residency, the interview process will continue with our Clinical Director followed by an interview with our Substance Abuse Counselor.
After completion of all 3 interviews, the Sojourners' Place Case Management team will make a final determination whether to extend an offer of acceptance. In most cases our decision will be finalized and available the same day. Prospects who are not accepted will be provided with contact information regarding alternative housing.
Once an offer of residency is made, prospects must report, clean and sober, to Sojourners' Place on the day, and at the time, determined by our Case Management team.
And, from there, the journey begins!
